Student Inclusion Form

I created this form with the help of a Japanese colleague as the International Baccalaureate was relatively new in working in the Japanese language during my time working there.

The form includes the following

• Information on the student.
• Test taken.
• Standardised scores.
• Areas where the student faces a challenge.
• Doctor`s recommendations on how the International Baccalaureate can support the student with their studies.
• The details on the form are from the IB`s Candidates with Assessment Requirements Document. As a school we got this form translated into Japanese and shared it with the International Baccalaureate.
